Advocates is uniquely positioned to offer cost-effective, person-centered, and results-driven care management for clients with behavioral health needs, many of whom have complex medical conditions. For forty years, our outpatient and residential behavioral health services have helped individuals achieve their goals through care coordination, connecting them to a network of support in the community.
Today, Integrated Care Management is one of our foremost areas of growth. We function as a health home, offering both direct behavioral health services and care management in an integrated fashion. We operate programs in partnership with Medicare/Medicaid OneCare, other MassHealth payers, and commercial health plans. Our programs are designed to promote recovery and wellness, to increase utilization of health services and community resources, and to reduce hospitalizations, emergency department visits, and other intensive services.
Our interdisciplinary Integrated Care Management team is led by a clinical social worker with more than 30 years of experience in care management and includes masters-level licensed care managers, peers, nurses, and bachelors-level care coordinators. Staff members are mobile and committed to providing flexible supports to meet the specific needs of each participant.
