Advocates’ Behavioral Health Supports for Justice Involved Individuals (BH-JI) program offers transitional case management, care coordination, and community support services for justice-involved adults who are returning to, or residing in, Middlesex or Essex Counties.
Our skilled team of reentry navigators is available to help you identify and access local providers, community resources, and wraparound supports.
Reentry Navigators provide assistance with…
- Day of release planning and support transitioning back into the community
- Applying for or reactivating MassHealth, SNAP, and Social Security benefits
- Scheduling medical and behavioral health appointments and coordinating transportation
- Accessing substance use treatment, recovery supports, and sober housing options
- Obtaining critical necessities (food, emergency shelter, clothing, toiletries, identification, etc.)
- Finding and securing employment, vocational training, and/or continuing education opportunities
- Connection to community resources and supports
Eligibility
Eligible adults must meet the following requirements:
- Eligible for, or enrolled in, a MassHealth plan
- Justice-involved
- Expected to release from a correctional
institution in the next 6 months, or - Was released from a correctional institution within
the past 12 months, or - Is currently supervised by probation and/or parole
- Expected to release from a correctional
- Living in or releasing to a community within Middlesex or Essex County
- Have mental health and/or substance use needs
